Kalathra dosham is a dosham of the 7th house of the jathagam, the house of the husband or wife (kalathra sthanam). Jothidars say it is present when malefic planets such as Sani, Sevvai, Rahu, Kethu or Suriyan sit in or aspect the 7th house, or when the lord of the 7th house is weak or afflicted.
In matching, a kalathra dosham chart is usually paired with a chart of similar strength, the same way sevvai dosham is matched with sevvai dosham.
There is no single fixed rule. Different jothidars and different astrology books use slightly different lists, so the same jathagam can be read as "kalathra dosham undu" by one and "mild" by another.
What Is the Meaning of Kalathra Dosham in Tamil?
Kalathram (களத்திரம்) means wife or spouse. Kalathra dosham is "the dosham of the spouse's house". Tamil families also call the 7th house the kalathra sthanam or kalathra bhavam.
So when a jothidar in Kumbakonam says "kalathra dosham irukku", he means the 7th house in that chart has a problem he wants to compare with the other chart before he approves the match.
How Do Jothidars Check Kalathra Dosham?
Most start from the 7th house counted from the lagnam, then look at the same house from the moon (chandra lagnam). The points they check:

Some jothidars widen the rule to malefics in the 1st, 2nd, 4th, 7th, 8th or 12th house, close to the sevvai dosham house list. Others keep it strictly to the 7th. Astrology websites show the same split, so do not be surprised if two printouts disagree.
Most jothidars also look for softening factors before they call it a dosham: Guru (Jupiter) or Sukran aspecting the 7th house, or a strong 7th lord in a good house.
Kalathra Dosham for Female Charts
The basic rule is the same for a girl and a boy. The 7th house and its lord are read the same way.
The difference some jothidars make is in the karaka, the planet that stands for the spouse. For a man's chart it is Sukran. For a woman's chart, many jothidars read Guru as the karaka for the husband, while some use Sevvai. Ask your jothidar which one he uses.
In practice, girls' families worry about this far more than boys' families. The rule itself does not say a girl's dosham is worse. A dosham in either chart is weighed the same way in matching.

How Is It Checked in Horoscope Matching?
The 10 porutham are counted from the stars of the boy and the girl. They do not look at the 7th house at all, so a match can get 7 or 8 poruthams and still have a kalathra dosham question.
That is why most Tamil jothidars check the full jathagam after the porutham count. They compare the malefic planets in both charts, a step called papa samyam.

None of this is a prediction of what will happen in a marriage. It is a method families use to choose, and jothidars themselves differ on it.
Kalathra Dosham Pariharam
Families with a kalathra dosham chart often do a parigaram (remedy) before or during the match search. The ones families mention most:

A parigaram is an act of faith. It does not change where the planets sit, and most jothidars will still look for a compatible chart afterwards. Be careful of anyone who asks for large sums with a promise that the dosham will "go away".
Writing Kalathra Dosham on a Biodata
Most Tamil families do not write kalathra dosham on the biodata at all. They share the jathagam, and the other side's jothidar reads it himself. Some families add a line like "Kalathra dosham: Illai" when they are sure, to save time.
